New RSV vaccines may soon put an end to rough seasons

It’s shaping up to be a severe season for respiratory syncytial virus infections – one of the worst some doctors say they can remember. But even as babies struggling to breathe fill hospital beds across the United States, there may be a light ahead: After decades of disappointment, four new RSV vaccines may be nearing review by the US Food and Drug Administration, and more than a dozen others are in testing.

There’s also hope around a promising long-acting injection designed to be given right after birth to protect infants from the virus for as long as six months. In a recent clinical trial, the antibody shot was 75% effective at heading off RSV infections that required medical attention.

Experts say the therapies look so promising, they could end bad RSV seasons as we know them.

And the relief could come soon: Dr. Ashish Jha, who leads the White House Covid-19 Response Task Force, told CNN that he’s “hopeful” there will be an RSV vaccine by next fall.

Charlotte Brown jumped at the chance to enroll her own son, a squawky, active 10-month-old named James, in one of the vaccine trials this summer.

“As soon as he qualified, we were like ‘absolutely, we are in,’ ” Brown said.

Babies have to be at least 6 months old to enter the trial, which is testing a vaccine developed at the National Institutes of Health – the result of decades of scientific research.

Brown is a pediatrician who cares for hospitalized children at Vanderbilt University Medical Center in Nashville, and she sees the ravages of RSV firsthand. A recent patient was in the back of her mind when she was signing up James for the study.

“I took care of a baby who was only a few months older than him and had had nine days of fever and was just absolutely pitiful and puny,” she said. Brown said his family felt helpless. “And I was like, ‘this is why we’re doing it. This single patient is why we’re doing this.’ “

Even before this year’s surge, RSV was the leading cause of infant hospitalizations in the US. The virus infects the lower lungs, where it causes a hacking cough and may lead to severe complications like pneumonia and inflammation of the tiny airways in the lungs called bronchiolitis.

Worldwide, RSV causes about 33 million infections in children under the age of 5 and hospitalizes 3.6 million annually. Nearly a quarter-million young children die each year from complications of their infections.

RSV also preys on seniors, leading to an estimated 159,000 hospitalizations and about 10,000 deaths a year in adults 65 and over, a burden roughly on par with influenza.

Despite this heavy toll, doctors haven’t had any new tools to head off RSV for more than two decades. The last therapy approved was in 1998. The monoclonal antibody, Synagis, is given monthly during RSV season to protect preemies and other high-risk babies.

The hunt for an effective way to protect against RSV stalled for decades after two children died in a disastrous vaccine trial in the 1960s.

That study tested a vaccine made with an RSV virus that had been chemically treated to render it inert and mixed with an ingredient called alum, to wake up the immune system and help it respond.

It was tested at clinical trial sites in the US between 1966 and 1968.

At first, everything looked good. The vaccine was tested in animals, who tolerated it well, and then given to children, who also appeared to respond well.

“Unfortunately, that fall, when RSV season started, many of the children that were vaccinated required hospitalization and got more severe RSV disease than what would have normally occurred,” said Steven Varga, a professor of microbiology and immunology at the University of Iowa, who has been studying RSV for more than 20 years and is developing a nanoparticle vaccine against the virus.

A study published on the trial found that 80% of the vaccinated children who caught RSV later required hospitalization, compared with only 5% of the children who got a placebo. Two of the babies who had participated in the trial died.

The outcomes of the trial were a seismic shock to vaccine science. Efforts to develop new vaccines and treatments against RSV halted as researchers tried to untangle what went so wrong.

“The original vaccine studies were so devastatingly bad. They didn’t understand immunology well in those days, so everybody said ‘oh no, this ain’t gonna work.’ And it really was like it stopped things cold for 30, 40 years,” said Dr. Aaron Glatt, an infectious disease specialist at Mount Sinai South Nassau in New York.

Regulators re-evaluated the guardrails around clinical trials, putting new safety measures into place.

“It is in fact, in many ways, why we have some of the things that we have in place today to monitor vaccine safety,” Varga said.

Researchers at the clinical trial sites didn’t communicate with each other, Varga said, and so the US Food and Drug Administration put the publicly accessible Vaccine Adverse Events Reporting System into place. Now, when an adverse event is reported at one clinical trial site, other sites are notified.

Another problem turned out to be how the vaccine was made.

Proteins are three-dimensional structures. They are made of chains of building blocks called amino acids that fold into complex shapes, and their shapes determine how they work.

In the failed RSV vaccine trial, the chemical the researchers used to deactivate the virus denatured its proteins – essentially flattening them.

“Now you have a long sheet of acids but no more beautiful shapes,” said Ulla Buchholz, chief of the RNA Viruses Section at the National Institutes of Allergy and Infectious Diseases.

“Everything that the immune system needs to form neutralizing antibodies that can block and block attachment and entry of this virus to the cell had been destroyed in that vaccine,” said Buchholz, who designed the RSV vaccine for toddlers that’s being tested at Vanderbilt and other US sites.

In the 1960s trial, the kids still made antibodies to the flattened viral proteins, but they were distorted. When the actual virus came along, these antibodies didn’t work as intended. Not only did they fail to recognize or block the virus, they triggered a powerful misdirected immune response that made the children much sicker, a phenomenon called antibody-dependent enhancement of disease.

The investigators hadn’t spotted the enhancement in animal studies, Varga says, because the vaccinated animals weren’t later challenged with the live virus.

“So of course, we require now extensive animal testing of new vaccines before they’re ever put into humans, again, for that very reason of making sure that there aren’t early signs that a vaccine will be problematic,” Varga said.

About 10 years ago, a team of researchers at the NIH – some of the same investigators who developed the first Covid-19 vaccines – reported what would turn out to be a pivotal advance.

They had isolated the structure of the virus’s F-protein, the site that lets it dock onto human cells. Normally, the F-protein flips back and forth, changing shapes after it attaches to a cell. The NIH researchers figured out to how freeze the F-protein into the shape it takes before it fuses with a cell.

This protein, when locked into place, allows the immune system to recognize the virus in the form it’s in when it first enters the body – and develop strong antibodies against it.

“The companies coming forward now, for the most part, are taking advantage of that discovery,” said Dr. Phil Dormitzer, a senior vice president of vaccine development at GlaxoSmithKline. “And now we have this new generation of vaccine candidates that perform far better than the old generation.”

The first vaccines up for FDA review will be given to adults: seniors and pregnant woman. Vaccination in pregnancy is meant to ultimately protect newborns – a group particularly vulnerable to the virus – via antibodies that cross the placenta.

Vaccines for children are a bit farther behind in development but moving through the pipeline, too.

Four companies have RSV vaccines for adults in the final phases of human trials: Pfizer and GSK are testing vaccines for pregnant women as well as seniors. Janssen and Bavarian Nordic are developing shots for seniors.

Pfizer and GSK use protein subunit vaccines, a more traditional kind of vaccine technology. Two other companies build on innovations made during the pandemic: Janssen – the vaccine division of Johnson & Johnson – relies on an adenoviral vector, the same kind of system that’s used in its Covid-19 vaccine, and Moderna has a vaccine for RSV in Phase 2 trials that uses mRNA technology.

So far, early results shared by some companies are promising. Janssen, Pfizer and GSK each appear effective at preventing infections in adults for the first RSV season after the vaccine.

In an August news release, Annaliesa Anderson, Pfizer’s chief scientific officer of Vaccine Research and Development, said she was “delighted” with the results. The company plans to submit its data to the FDA for approval this fall.

GSK has also wrapped up its Phase 3 trial for seniors. It recently presented the results at a medical conference, but full data hasn’t been peer reviewed or published in a medical journal. Early results show that this vaccine is 83% effective at preventing disease in the lower lungs of adults 60 and older. It appears to be even more protective – 94% – for severe RSV disease in those over 70 and those with underlying medical conditions.

“We are very pleased with these results,” Dormitzer told CNN. He said the company was moving “with all due haste” to get its results to the FDA for review.

“We’re confident enough that we’ve started manufacturing the actual commercial launch materials. So we have the bulk vaccine actually in the refrigerator, ready to supply when we are licensed,” he said.

Even as the company applies for licensure, GSK’s trial will continue for two more RSV seasons. Half the group getting the vaccine will be followed with no additional shots, while the other group will get annual boosters. The aim is to see which approach is most protective to guide future vaccination strategies.

Janssen’s vaccine for older adults appears to be about 70% to 80% effective in clinical trials so far, the company announced in December.

In a study on Pfizer’s vaccine for pregnant women published in the New England Journal of Medicine this year, the company reported that the mothers enrolled in the study made antibodies to the vaccine and that these antibodies crossed the placenta and were detected in umbilical cord blood just after birth.

The vaccines for pregnant women are meant to get newborns through their first RSV season. But not all newborns will benefit from those. Most maternal antibodies are passed to baby in the third trimester, so preemies may not be protected, even if mom gets the vaccine.

For vulnerable infants and those whose mothers decline to be vaccinated, Dr. Helen Chu, an infectious disease specialist at the University of Washington, says the long-acting antibody shot for newborns, called nirsevimab, should cover them for the first six months of life. She expects it to be a “game-changer.”

That shot, which has been developed by AstraZeneca, was recently recommended for approval in the European Union. It has not yet been approved in the United States.

The field is so close to a new approval that public health officials say they’ve been asked to study up on the data.

Chu, who is also a member of an RSV study group of the Advisory Committee on Immunization Practices, a panel that advises the US Centers for Disease Control and Prevention on its vaccine recommendations, says her group has started to evaluate the new vaccines – a sign that an FDA review is just around the corner.

No companies have yet announced that process is underway. FDA reviews can take several months, and then there are typically discussions and votes by FDA and CDC advisory groups before vaccines are made available.

“We’ve been working on this for several months now to start reviewing the data,” Chu said. “So I think this is imminent.”

Watching this year’s RSV season unfold, Brown, the pediatrician who enrolled her son in the vaccine trial for toddlers, says progress can’t come fast enough.

“The hospital is surging. We’re not drowning the way some states are. I mean, Connecticut, South Carolina, North Carolina, they’re really drowning. But our numbers are huge, and our services are so busy,” she says.

Brown says her son is mostly healthy. He doesn’t have any of the risks for severe RSV she sees with some of her patients, so she was happy to have a way to help others.

And while it’s far too early to say whether the vaccine James is helping to test will prove to be effective, the trial was unblinded last week, and Brown learned that her son was in the group that got the active vaccine, not the placebo

He has done well through this heavy season of illness, she says. The NIH-sponsored study they participated in is scheduled to be completed next year.

The vaccine, which is made with a live but very weak version of virus, is given through a couple of squirts up the nose, so there are no needles. The hardest part for squirmy James, she said, was being held still.

“If we can do anything to move science forward and help another child, like, sorry, James. You had to have your blood drawn, but it absolutely was worth it.”

“It’s going to be a rough winter”: Hospitals overwhelmed by pediatric patients with respiratory virus

Hospitals in 33 states are seeing a dramatic rise in children suffering from the respiratory virus called respiratory syncytial virus (RSV). Cases have more than doubled in 25 states, putting a strain on hospitals — with some facilities so overwhelmed, they’re running out of beds.

At Connecticut Children’s hospital, doctors said they’re slammed with a surge in RSV cases. Dr. John Brancato told CBS News that every inch of the emergency room is filled, and the hospital is considering putting a tent on the front lawn to handle the overflow.

The state of Connecticut is even thinking of bringing in the National Guard.

“We’re having patients in hall beds,” Brancato said. “We’re using our orthopedic room. We’re using other treatment rooms as much as possible to take care of everybody.”

RSV cases typically surge from December to February, but this month, the children’s hospital has more RSV cases than any other respiratory illness, including COVID-19.

Further south, almost half of the ICU beds at Cook Children’s Hospital in Fort Worth, Texas, are filled with RSV patients.

“It’s going to be a rough winter,” Dr. Daniel Guzman, who works at the hospital, told CBS News. “I mean, we’re already seeing our numbers spike over the last few weeks with over 550 E.R. visits per day.”

Parents Zoe and Jeff Green said their 4-month-old daughter, Lindy, is in the ICU with RSV. They took her to an urgent care clinic because they were concerned that she had more than just a common cold.

RSV symptoms are similar to a cold, but doctors said parents should watch for signs of respiratory stress, if their child’s nostrils are flaring while breathing, or if their skin is pulling towards their ribs.

Doctors are also expecting an active flu season. They’re asking people to get their flu shots now — while it won’t prevent you from getting the flu, it could make your symptoms mild enough where you wouldn’t need to go to the emergency room.

Flu ‘gathering speed’ across US as health officials prepare for potentially rough season

US health officials are becoming increasingly concerned about this year’s flu season – and are already seeing signs that the virus is spreading.

As the 2022-23 flu season gets underway, one high school in California is facing a “high number of absences” among students due to possible flu cases. Flu activity in the United States often starts to increase in October and usually peaks between December and February.

“We can confirm that there is a high number of absences at Henry High School due to probable Influenza,” Samer Naji, a spokesperson for the San Diego Unified School District, said in an email to CNN on Thursday. There were about 1,000 absences Wednesday, out of 2,600 students.

“So far COVID tests have been negative but several students have tested positive for flu,” Naji said. “Typical signs and symptoms include cough, sore throat, runny nose, fever, and other upper respiratory infection symptoms. We are in close contact with San Diego County Public Health.”

San Diego County Public Health Services confirmed to CNN that it is investigating the large outbreak of respiratory and flu-like symptoms among students at Patrick Henry High School – as well as Del Norte High School in Poway, California.

“Del Norte High School (in the Poway Unified School District) had nearly 400 students absent yesterday and Tuesday with cold and flu like symptoms,” Christine Paik, spokesperson at Poway Unified School District, told CNN on Wednesday. “Health officials have told us that the cold/flu season is definitely here and it’s hitting schools harder now that COVID restrictions are no longer in place.”

San Diego County Public Health Services announced Wednesday that “it is too early” to determine the cause of the outbreaks and that the county is evaluating.

“We are coordinating with local school districts and are checking with other school campuses to try and figure out why so many students have been affected so suddenly,” Dr. Cameron Kaiser, the county’s deputy public health officer, said in an announcement Wednesday.

“Unfortunately, we anticipated this would be a rough influenza season, and alongside COVID-19 other respiratory viruses are also making a rapid comeback,” Kaiser said. “If you haven’t already, now is the time to get your flu and COVID-19 shots to gain the extra protections afforded by the vaccines.”

An early increase in seasonal flu activity has been reported in most of the United States, with the nation’s southeast and south-central areas reporting the highest levels of flu, according to the US Centers for Disease Control and Prevention. More than a thousand patients have been hospitalized with flu this week, the agency said.

“Nationally, the percentage of specimens testing positive for influenza in clinical laboratories is increasing; however, activity varies by region,” CDC researchers wrote in the agency’s latest weekly flu report, published Friday.

While current influenza activity is still overall low, the CDC’s report finds that activity is increasing in most of the country, with three jurisdictions experiencing moderate activity and six jurisdictions experiencing high or very high activity.

This week, 3.3% of respiratory specimens sent to clinical labs tested positive for influenza, according to the new report. That is a jump from the 0.1% of specimens that tested positive this time last year, and the 0.2% of specimens that tested positive this time in 2020.

However, the new data suggest the nation might be returning to pre-Covid levels of flu, as around this time in 2019, 3.1% of specimens were reported as testing positive for flu.

The new CDC report also said that 1,322 patients were admitted to hospitals with influenza this week and there were three deaths in children this week associated with influenza.

Overall, the most frequently reported flu viruses this week were influenza A (H3N2), according to the report, and 2.6% of outpatient visits to health care providers were for respiratory illnesses that included symptoms such as fever plus a cough or sore throat. That is above the national baseline, which is 2.5%.

“An annual flu vaccine is the best way to protect against flu. Vaccination helps prevent infection and can also prevent serious outcomes in people who get vaccinated but still get sick with flu,” according to the report.

“CDC recommends that everyone ages 6 months and older get a flu vaccine, ideally by the end of October. There are also prescription flu antiviral drugs that can be used to treat flu illness; those need to be started as early as possible.”

Predicting what influenza activity might look like in a particular year can be tricky, but doctors are bracing for “a very substantial” flu season, said Dr. William Schaffner, a professor in the Division of Infectious Diseases at Vanderbilt University Medical Center and medical director of the National Foundation for Infectious Diseases.

“I’m concerned that we will have a very substantial influenza season coming up this year, very different from our two previous seasons,” he said.

Signs of increased flu activity were first seen in the Southern Hemisphere this summer, and as more people in the Northern Hemisphere relax Covid-19 restrictions and return to socializing without masks and in large crowds, cases of flu are being reported. The widespread number of cases so early in the flu season is unusual.

“Here we are in the middle of October – not the middle of November – we’re already seeing scattered influenza cases, even hospitalized influenza cases, around the country,” Schaffner, whose medical center at Vanderbilt University is part of a surveillance network that tracks hospitalized flu cases.

“So we know that this virus is now spreading out in the community already. It’s gathering speed already. It looks to me to be about a month early,” Schaffner said.

When people began isolating, social distancing and masking to slow the spread of Covid-19 in early 2020, flu all but disappeared in the US. As a result, most people haven’t been exposed to influenza for a few years, meaning immunity against flu viruses could be low and underscoring the need to get vaccinated.

US health officials are encouraging people to get their flu shots as soon as possible, said Adriane Casalotti, chief of government and public affairs for the National Association of County and City Health Officials.

“There’s definitely a push now to get flu vaccination to the top of people’s priority lists. We’re all concerned, having seen what’s going on in the Southern Hemisphere, in Australia, that this year’s flu season could be really, really rough, frankly,” Casalotti said.

Concern has grown as officials also brace for possible surges this winter in Covid-19 and other common respiratory viruses, such as respiratory syncytial virus or RSV, a leading cause of lower respiratory tract infection in infants and young children.

Casalotti said that the spread of influenza can have a significant effects on communities, especially since it can be hard to distinguish flu symptoms from those of Covid-19, common colds or allergies.

“We’re already starting to see in some areas that flu is circulating,” Casalotti said. “Overall, flu activity is low nationwide, but it’s starting to tick up in the Southeast in particular.”

In August, as concern about the coming flu season grew, the US Centers for Disease Control and Prevention’s Advisory Committee on Immunization Practices released a report updating its recommendations for the seasonal flu shot for this year.

Typically, older adults get a higher dose of flu vaccine than younger adults, but in the latest update, ACIP recommended that adults 65 and older “preferentially receive” a higher dose or an adjuvanted flu vaccine.

“There are three of the influenza vaccines that have been shown to work better in people aged 65 and older,” said Schaffner, who is a liaison representative to ACIP. “There is the high-dose vaccine, there’s another that has an adjuvant in it – an immune stimulant – and the third is a recombinant vaccine.” Recombinant flu vaccines do not involve the flu virus or chicken eggs in the manufacturing process.

“If you look back at people aged 65 and older in previous seasons, already 80% of them were receiving one of these three vaccines,” he said. “What’s new this year is that the ACIP has made a distinctively preferential recommendation. They actually said if you’re vaccinating people aged 65 and older, preferentially use one of these three vaccines, and only if one of them is not available use the regular vaccine.”

Overall, the CDC recommends that everyone 6 months and older get an annual flu vaccine, especially people who could be pregnant – as the vaccine offers protection not only to them but to their baby, Schaffner said.

“There’s an additional benefit,” he said. “Namely, the antibodies created in response to the vaccine, some of it crosses the placenta and goes into the newborn baby. So the newborn baby gets some of that protection during the first four to six months of its life, before we can actively vaccinate the baby.”

How bad will flu and Covid be this winter? Hospitals brace for rough season.

Hospitals nationwide are preparing for another winter with Covid — the first one that’s also expected to include high levels of influenza and other respiratory illnesses that have simmered quietly in the background for the past two years.

Flu cases are already rising in parts of the U.S., according to the Centers for Disease Control and Prevention. Pediatricians, too, are seeing a growing number of children sick with respiratory syncytial virus, or RSV, and enteroviruses.

And despite a downward trend in Covid, tens of thousands of new cases are still being diagnosed every day.

The convergence of viruses is hitting health care systems as they’re forced to reckon with staffing shortages that worsened during the pandemic.

“If you go around the nation and ask hospitals how busy they are, every single one of them will tell you: They’re busy,” said Dr. Carlos del Rio, an executive associate dean at the Emory University School of Medicine and Grady Health System in Atlanta.

Health care workers are quitting at rates 23 percent higher than when the pandemic began, mirroring a larger nationwide trend of workers leaving their jobs, according to Health System Tracker, a joint effort between the nonprofits Peterson Center on Healthcare and Kaiser Family Foundation to monitor how well the U.S. health care system is performing.

“Nurses were on the front line, and some of them burned out and quit,” said Dr. James McDeavitt, executive vice president and dean of clinical affairs at Baylor College of Medicine in Houston. “Others that were in their 50s and 60s who maybe thought they’d be working for another five years took an early retirement.”

Dr. Bernard Camins, medical director for infection prevention at the Mount Sinai Health System in New York City, has noted a similar “mass exodus” of health care workers who retired early or moved to a different line of work altogether.

Now, he said, “there’s a constant struggle to recruit new people.”

Staffing deficits mean there is little wiggle room to accommodate any additional surges of patients, whether they’re sick with Covid, flu or other illness.

“There’s no excess capacity in hospitals,” del Rio said. “Anything that increases the number of patients is going to tip the scales.”

Despite the shortages, hospital leaders applaud health care workers who have been able to stay the course and are ready for this next phase of infectious disease.

Morale is “actually pretty good,” McDeavitt said. “We’ve moved on from early in the pandemic, wondering if we were going to get sick and potentially die.”

“I think those worries are alleviated,” McDeavitt said. “We know how to handle it now.”

Where are we now?

Reports of Covid cases have been decreasing steadily since early August. As of Oct. 6, the average number of new cases per day, based on a seven-day average, is 44,743 — the lowest it’s been since April.

Covid-related hospitalizations, too, continue to tick down. As of Oct. 5, the average daily number of hospital beds used by a Covid patient was at its lowest since June, at 27,161.

But as the cold weather sets in and people increasingly gather indoors, Covid cases are expected to rise.

A recent analysis from the Mayo Clinic in Rochester, Minnesota, predicts that average Covid cases per day nationwide will increase by more than 10 percent in the coming weeks.

“We’ll see an increase in Covid cases — probably not to the extent that we saw in the winter of 2020 and winter of 2021 — but we will see a rise,” said Matthew Binnicker, director of clinical virology at the Mayo Clinic. “Most of those will be infections that lead to mild to moderate illness.”

How well do Covid boosters work?

That prediction reflects what is known so far about how the latest Covid vaccines work. While the shots may not prevent a person from getting sick following an infection, they have been shown throughout the pandemic to keep infected people out of the hospital and from dying.

“How much of a rise in the hospitalizations and deaths we will see is really going to depend on the extent to which people are getting up to date on their vaccines,” especially those at highest risk for severe illness, such as the elderly and those with weak immune systems, said Jennifer Nuzzo, director of the Pandemic Center at Brown University School of Public Health.

The vast majority of Covid cases circulating now are an omicron subvariant, BA.5. The latest version of the Covid vaccine targets BA.5, but since its debut in September, fewer than 4% of people eligible for the extra shot have received it.

The Commonwealth Fund recently predicted that more than 745,000 Covid-related hospitalizations and more than 75,000 such deaths could be avoided, if more people got the bivalent shot.

The U.S. is also seeing the beginning of what is expected to be the first rough flu season in years. While overall numbers remain low, the CDC reported an increase in positive flu tests last week.

“It’s very likely that we’re going to see influenza roaring back with a vengeance this winter,” said Dr. Dan Uslan, co-chief infection prevention officer for UCLA Health in Los Angeles.

What’s happening in Australia could be a preview: The country is exiting its worst flu season in at least five years, according to the country’s Department of Health and Aged Care.

“Data from the Southern Hemisphere are not good,” Binnicker, of the Mayo Clinic, said. “We need to double down on prevention measures,” such as masking and physical distancing.

There are already signs that viruses are circulating more than they have in recent years.

Pediatricians have begun to see “high numbers of severely sick patients with respiratory illnesses,” said Dr. Sarah Combs, an emergency medicine physician at Children’s National in Washington, D.C. The illnesses are not necessarily linked to either Covid or flu.

“We are seeing child after child coming in with breathing trouble related to what would commonly be called just a cold or a sniffle,” Combs said. These are children who do not have asthma or other chronic lung diseases that would make them more susceptible to breathing problems.

“They come in having a cold and then within 24 hours they’re actually struggling to breathe,” she said.

The CDC recommends an annual flu shot for everyone aged 6 months and older. Children younger than age 9 who have never had a flu vaccine, the CDC said, should get two this year, at least four weeks apart.
