COVID NY Update: State runs out of COVID vaccine, using 2nd doses as 1st doses up to federal government, Cuomo says

NEW YORK CITY (WABC) — Both New York City and New York state have run completely dry of covid vaccines with the next shipment not expected until early next week.

The news comes as more and more hospitals cancel vaccination appointments with no new appointments scheduled through Monday.

15 vaccine centers have also closed.

Gov. Andrew Cuomo is blaming the now-former Trump administration for opening up too many categories of eligibility, without increasing supply.

“It should have been opened as you had allocation. Anyway, that is not what we did, it’s not what they did. And now you have a period of confusion and anxiety because you’re trying to hit seven million people at 250,000 a week,” Cuomo said.

Another quarter-million doses will arrive in time for next week, but state officials say that’s far from enough.

Some have suggested using second doses, which are in storage, as first doses.

During a visit to a COVID-19 vaccine pop-up site at a NYCHA complex in Brooklyn Saturday, Cuomo said that would only be possible with collaboration from the federal government.

“The reason why the federal government has to agree is because they have to agree that they’re then going to send you enough second doses in the future. Otherwise, you want them giving people first dosages and then you don’t have a second dosage for them,” Cuomo said.

Health experts say administering the second dose of the vaccine within the timeframe outlined by federal guidelines is critical to ensuring long-term protection.

So far more than one million people in the state have gotten their first dose of the vaccine.

At a Friday briefing, Cuomo said deliveries of the week six allocations, about 250,400 doses, are on the way, but those deliveries “are delivered by the federal government by various means and they arrive at different times throughout the week.”

The state is able to administer about 80,000 doses per day and could easily do 100,000 per day, Cuomo said.

“The moment the vaccines arrive, our goal is to get them in arms as soon as possible. 250,400 doses per week is not enough,” Cuomo said.

Biden stresses COVID, immigration in first calls with foreign leaders

President Joe Biden stressed North American cooperation on the coronavirus pandemic, climate change and immigration in his first telephone calls with Mexican and Canadian leaders.

In phone calls Friday with Canadian Prime Minister Justin Trudeau and Mexican President Andres Manuel Lopez Obrador, Biden promised to strengthen regional cooperation, according to reports.

In the call with Trudeau, the first foreign leader to speak with the new president, the two leaders “discussed collaboration on vaccines and acknowledged that the two countries’ efforts are strengthened by existing exchanges of medical personnel and the flow of critical medical supplies,” according to Canadian reports.

Although Trudeau hailed Biden’s presidency as a “new era” for relations between the countries, he complained that Biden had scrapped an oil pipeline linking the two countries on his first day in office. According to a White House statement, Biden acknowledged “Trudeau’s disappointment regarding the decision to rescind the permit for the Keystone XL pipeline.”

In the conversation with Lopez Obrador, the Mexican president addressed the contribution of Mexican migrants in the US and said the best way to manage migration was to create economic development in impoverished regions from which migrants set off, according to a statement from Mexico’s foreign ministry.

The call comes at a time of tension over the US federal investigation into former Mexican defense minister Salvador Cienfuegos, which was dropped in November. US prosecutors had claimed that Cienfuegos was the head of the H-2 drug cartel.

There is some evidence that a new coronavirus variant first identified in southeast England carries a higher risk of death than the original strain, the British government’s chief scientific adviser said Friday — though he stressed that the data is uncertain. Patrick Vallance told a news conference that “there is evidence that there is an increased risk for those who have the new variant.” He said that for a man in his 60s with the original version of the virus, “the average risk is that for 1,000 people who got infected, roughly 10 would be expected to unfortunately die.”

“With the new variant, for 1,000 people infected, roughly 13 or 14 people might be expected to die,” he said.

Researchers developing face mask sticker that can detect COVID-19 in droplets
Researchers at UC San Diego’s School of Engineering are working on a potential game changer in the fight against COVID-19. Researchers are looking into a new type of test that could detect the virus on your face mask. The test can be worn as a sticker on your mask. The sticker includes a little dye and works just like an at-home pregnancy test. As someone wears the sticker on the mask throughout the day, it collects droplets. After a few hours, it can detect COVID-19 molecules from your breath.

Dave Chappelle tests positive for COVID-19
Dave Chappelle tested positive for the coronavirus just before his comedy show scheduled for Thursday, forcing his upcoming appearances to be canceled, a spokeswoman said. Chappelle was expected to perform Thursday through Sunday at Stubb’s Waller Creek Amphitheater in Austin, Texas. Those shows have been canceled and Chappelle is quarantining, his representative Carla Sims said in a statement. The comedian is asymptomatic. Chappelle had been performing socially-distanced shows in Ohio since June, and moved his shows to Austin during the winter, Sims said. Rapid testing for the audience and daily tests for Chappelle and his team were implemented.

The state of New Jersey has vaccinated 500,222 residents in a little more than a month, Governor Phil Murphy announced on Friday. He also added that two COVID patients in the state were found to have the more aggressive strain first found in the UK.

NY hospitalization rate of increase is slowing
The rate of increase of patients being admitted to New York hospitals for treatment for COVID-19 is slowing, Gov. Andrew Cuomo said Friday. Cuomo called it “good news,” but cautioned that the more contagious variants discovered in other parts of the world remain a threat and could cause cases to spike again. COVID hospitalizations stood at 8,846, down from 9,055 the day before and has now dropped by more than 400 over the past two days.

NYC and state could run out of vaccines Friday
93% of the state’s vaccine supply now exhausted, both the city and the state are on pace to run out of vaccines.
There are 300,000 shots waiting in storage for second doses.

If This Sounds Like You, You’re at Risk for Severe COVID, Says Doctor 

One of the most curious aspects of COVID-19 is that when infected, some people remain completely asymptomatic, while others end up hospitalized and unable to breathe. Since the start of the pandemic, health experts and researchers have been studying the highly infectious virus in hopes of understanding why the type of infections varies so dramatically on a person-by-person basis. According to one infectious disease expert, it boils down to three factors. The first and only controllable part of the equation is how the individual is infected and the dose of the virus they receive, says Carlos R. Oliveira, MD, Ph.D., Yale Medicine pediatric infectious disease doctor. “For example, if an individual infected with COVID-19 coughs directly on you, you will likely get a much higher dose of the virus than if you are infected by touching a contaminated surface,” he points out. “A higher infectious dose usually leads to more rapid onset and more severe symptoms.” 

This is a prime example of why wearing a mask and social distancing is so crucial in protecting yourself and others. Multiple studies have concluded that masks are effective in preventing tiny, infected viral particles from transmitting from person-to-person. While they might not stop all of them, they will certainly help reduce the viral load, likely resulting in a less serious infection.

Another crucial factor in determining the type and severity of COVID symptoms is “the health status at the time of infection,” according to Dr. Oliveira. Early on in the pandemic, it became clear that people with certain pre-existing conditions were more prone to severe infection than others. 

According to the CDC, anyone who is suffering from cancer, chronic kidney disease, COPD (chronic obstructive pulmonary disease), Down Syndrome, heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies, an immunocompromised state (weakened immune system) from solid organ transplant, obesity (body mass index [BMI] of 30 kg/m2 or higher but < 40 kg/m2), severe Obesity (BMI ≥ 40 kg/m2), sickle cell disease, type 2 diabetes mellitus, are pregnant, or a smoker, are at the highest risk of severe infection. 

“It is especially important for people at increased risk of severe illness from COVID-19, and those who live with them, to protect themselves from getting COVID-19,” the CDC warns.

Age is also a very important factor, according to Dr. Oliveira, influencing not only the severity of the virus but the symptoms it can manifest. “Several studies (including Agyeman, Mayo Clinic, 2020) have shown that in adults, loss of smell is a common early manifestation, occurring in up to 40-60% of infections. It may also be the only manifestation of infection in some adults,” he points out. 

In contrast, loss of smell is rarely seen in children, he adds, citing the COVID Symptom Study. “In fact, gastrointestinal symptoms, like abdominal pain, loss of appetite, and diarrhea, are frequent initial manifestations in children, occurring in about 1 in 3 cases.”

“As is the case for many infectious diseases, the progression of discernible symptoms can be quite variable,” Dr. Oliveira says.

1,900 COVID vaccine doses ruined at Boston VA hospital after freezer accidentally unplugged

Nearly 2,000 doses of a COVID-19 vaccine were spoiled at a Veterans Affairs hospital in Boston after a contractor accidentally unplugged a freezer, hospital officials announced Thursday. Staff at the Jamaica Plain VA Medical Center discovered on Tuesday that a freezer had failed, compromising 1,900 doses of the Moderna COVID-19 vaccine.

The plug to the freezer was found to be loose after a contractor accidentally unplugged it while cleaning, according to a statement from Kyle Toto, a spokesman for VA Boston Healthcare System. The freezer had been in a safe location and had an alarm system, he said.

Both Moderna and Pfizer-BioNTech’s vaccines require extremely cold temperatures for storage.  

“For the Moderna vaccine it’s 12 hours. Once it’s been at room temperature for longer than that you no longer can assure that it’s effective and so you can’t give the vaccine,” Dr. Paul Biddinger, the Medical Director for Emergency Preparedness at Mass General Brigham, told CBS Boston.

The system is investigating the cause of the incident and why the monitoring alarm system did not work. More doses are on the way, Toto said, and officials “do not foresee disruption” of the system’s vaccination effort.

Temperature issues have caused problems for vaccine rollouts in other states.

Nearly 12,000 Moderna doses that were being shipped to Michigan on Sunday were spoiled after getting too cold. In Wisconsin, a pharmacist faces charges after authorities say he deliberately ruined hundreds of doses by removing them from refrigeration for two nights.

The Moderna vaccine needs to be stored at regular freezer temperatures, but not the ultra-cold required for Pfizer-BioNTech’s shot.

CBS Boston reports Massachusetts Rep. Stephen Lynch said the doses have been moved to Brockton and West Roxbury while the cleanup operation is still ongoing.

“We simply believe it was an accident,” Lynch told reporters Friday. “Part of the contributing factor was the way these plugs operate. One of them is an offset, so it’s very difficult to pull out. But the one at the top of the freezer was a direct pull so the engineering staff here have corrected that. They’ve created a bracket, they’ve taken pictures of that plug and sent it around to all the other VA hospitals that have this thermo-scientific freezer so that in the event that might happen somewhere else.”

Fauci: Trump administration’s Covid strategy ‘very likely did’ cost lives

Former President Donald Trump faced significant criticism over the final year of his presidency for his pandemic response, which strayed often from the guidance of his own administration’s health officials and veered often into bizarre territory.

From the pandemic’s early stages, Trump regularly downplayed the risk Covid-19 posed to Americans and predicted often that the U.S. would soon have the virus beat, even as cases spiked around the country. He resisted the best practices recommended by his own public health team, declining to wear a mask in public and holding large-scale rallies, both indoors and outdoors, with thousands of supporters.

Trump frequently touted the malaria drug hydroxychloroquine as a treatment for Covid-19 despite there being no evidence of it being effective. At one point last spring, he suggested Americans should inject themselves with disinfectants to combat the virus.

Fauci’s regular appearances at White House coronavirus briefings made him a household name throughout the pandemic, but his relationship with Trump quickly soured as the NIAID chief refused to fall in line with the then-president’s inconsistent and at-times dangerous Covid-19 rhetoric. Trump called Fauci an idiot and a “disaster,” saying “if we listened to him, we’d have 700,000 [or] 800,000 deaths.” Trump accused Fauci and other health officials of exaggerating the pandemic’s severity and criticized officials for saying early on that masks weren’t necessary.

As the pandemic wore on, Fauci’s appearances at the White House grew increasingly rare, as did contact between the two men.

Fauci said in a news conference with White House press secretary Jen Psaki on Thursday that his work with the Biden administration thus far has been “liberating.” Fauci has said the Trump administration prevented him from making some media appearances and that the White House hampered the flow of public health information.

“You didn’t feel you could actually say something and there wouldn’t be repercussions about it,” Fauci said in the news conference Thursday. “One of the new things in this administration, is if you don’t know the answer, don’t guess. Just say you don’t know the answer.”

Fauci said in an interview Thursday that there is “complete transparency” under the Biden administration.

On CNN Friday, he also said that there was a coronavirus strategy under the Trump administration, but that it “wasn’t articulated well.”

“The separation of the federal government and the states … was really a lesion,” Fauci said on CNN. “You don’t want the federal government to do everything and you don’t want the states to do everything. … What we saw a lot of was saying ‘OK states, do what you want to do.’ And states were doing things that clearly were not the right direction.”

Biden’s Covid-19 strategy has included directing FEMA to establish Covid-19 liaisons to “maximize cooperation between the federal government and the states” and reimbursing states for using the National Guard in relief efforts.

“The best thing to do is to have a plan, have the federal government interact with the states in a synergistic, collaborative, cooperative way, helping them with resources and helping them with a plan, at the same time respecting the individual issues that any individual state might have,” Fauci said.

Covid vaccine: Experts say Biden’s strategy could benefit from good timing

“They’re going to take advantage of the learning curve,” said Vijay Samant, a former Merck executive who oversaw the production of three successful vaccines during his tenure. Samant said vaccine manufacturers have had months to work out supply bottlenecks and that manufacturing was always projected to speed up in the coming months — a boon for the Biden administration.

“They may get a lot of credit because all of a sudden vaccine doses are going to become available and it’s, ‘Oh, well, we did it,’ ” Samant said. “Let me tell you, that’s how it works. They’re on the right end of the curve.”

Connecticut Gov. Ned Lamont, a Democrat, sounded similarly optimistic Thursday, predicting that vaccine supply will rapidly increase in February and March. The governor was fresh off a call with Pfizer, during which the company “reiterated their plan to significantly ramp up production in February,” said the governor’s spokesman Max Reiss. Much of that ramp-up is taking place even before the Biden administration makes any moves under the Defense Production Act.

Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, appeared to agree with the assessment Thursday.

In an interview on ABC’s “Good Morning America,” Biden’s chief medical adviser expressed confidence in the President’s once seemingly far-reaching goal to vaccinate 100 million people in his first 100 days.

“I feel fairly confident that that’s going to be not only that but maybe even better,” Fauci said, adding that he had looked at the contractual agreements that had been made. “The amount that will be coming in, we will be able to meet that goal.”

Still, questions remain about where US vaccine supply currently stands. The new administration has touted robust federal plans to launch huge vaccination sites and deploy mobile units to hard-hit communities, at the same time some states are canceling appointments at existing vaccination sites, citing a lack of vaccine supply.

Biden’s team has said it’s flying somewhat blind when it comes to the supply of vaccine and how it has been allocated across the country. A source close to the transition told CNN that the Biden team had been denied access to critical resources it needed to accurately gauge the supply and prepare to take over the rollout before Biden took office.

“The cooperation or lack of cooperation funds from the Trump administration has been an impediment,” White House Covid-19 coordinator Jeff Zients told reporters. “So we don’t have the visibility that we would hope to have into supply and allocations.”

To be sure, there’s still not nearly enough vaccine available — in the US or worldwide — and the US has stumbled at getting shots into arms quickly. But the Biden administration also appears to be trying to manage expectations, playing up the shambles the Trump administration left behind while figuring out what concrete steps it can take to improve vaccine manufacturing and distribution.

The Biden administration has not clearly articulated what it’s doing to immediately improve vaccine supply, except to task agencies with looking for opportunities to use the Defense Production Act.

Biden “absolutely remains committed to invoking the Defense Production Act in order to get the supply and the materials needed to get the vaccine out to Americans across the country and remains committed to his goal of getting 100 million shots in the arms of Americans in the first 100 days,” White House press secretary Jen Psaki said Thursday.

The White House did not address how they would be communicating or working with states, but in a statement to CNN spokesman Kevin Munoz reiterated that they would be “listening to the unique needs of states and tailoring the federal government’s resources and guidance accordingly to ensure we are getting vaccines in the arms of Americans as quickly and safely as possible.”

White House officials said they have asked the Department of Health and Human Services to look for all potential sources of a type of syringe that can help extract more doses of the Pfizer vaccine from each vial. On Thursday, Biden said he would sign an executive order to use the Defense Production Act to direct federal agencies and private industries to “accelerate the making of everything that’s needed to protect, test, and vaccinate and the care of our people.”

A former administration official told CNN that the Trump administration had already implemented the Defense Production Act to assist each of the six vaccine manufacturers, as well as on needle and syringe contracts.

“We’ll be working paying close and careful attention to all of the components of the supply chain to ensure manufacturing keeps up with what we need,” Tim Manning, the White House Covid supply coordinator, told reporters. “The Defense Production Act is a powerful tool — powerful set of tools — and there are many ways we can use it to help ensure that there are adequate and growing supplies of vaccine.”

Samant, the vaccine expert, said the administration would be better off focusing on the availability of raw materials like lipids rather than specialized syringes.

“It’s like the most ridiculous thing to chase after,” Samant said of ramping up syringe production, adding that such a move might, at best, improve vaccine supply by 5%.

For the Biden team, it’s critical to understand how much supply is on hand and how much could realistically be available in the coming months. States have said they need clear and consistent guidance on vaccine supply so they can continue to streamline their vaccination programs.

“We will work to provide projections on supply. We hear over and over from governors and local leaders that they just don’t know what supply is coming and can’t plan. We will absolutely across the next few days to get our arms around what’s going on, make sure that we are communicating with states and localities, so they can prepare, effectively,” Zients added.

According to one health official involved in the vaccine distribution process, hospitals receiving the Moderna vaccine complained of being completely in the dark on when they were receiving it and how much they were getting and, in some cases, were sent scrambling when they received surprise shipments. Moderna declined to comment.

All these problems are now landing squarely in the lap of the new administration.

Some state officials said they’re already concerned about what the administration’s plan will be to allocate vaccines and communicate with states.

Under the previous administration, then-Vice President Mike Pence and members of the Covid task force held regular calls with the nation’s governors during most of the pandemic. While some governors found the calls largely useless, they could at least rely on the briefings as an opportunity to hear from high-ranking medical experts and ask questions of the administration.

A day after Biden took office, state officials told CNN there is still a lot of confusion as to how states will be working and communicating with the Biden administration as the pandemic continues to surge and vaccine distribution is woefully behind.

According to one Republican state official, the National Governors Association has reached out to some of its governors about working with the Biden administration on Covid. However, not all governors are members of the organization and it remains unclear how the administration plans to work with those outside the association.

A Democratic state official said they, too, had not had any regular communication with the new administration.

Dr. Bechara Choucair, the White House vaccinations coordinator, has been in touch with some governors but had not laid out how the administration would tackle vaccine allocations.

Biden said Thursday that each state would be assigned a liaison with the Federal Emergency Management Agency.

But in a news briefing Thursday, Psaki couldn’t offer any more specifics on how the Biden administration will improve communications with states.

The new Covid team, she said, “will be engaging with governors, Democrats and Republicans, mayors, local elected officials, to gain a better understanding of what’s happening on the ground.”

TIMELINE: NOTABLE DATES

  • Certain groups – including people 65 years of age or older – can get vaccinated as soon as Jan. 26. Vaccination scheduling begins Jan. 22.
  • On Jan. 5, the state released an updated vaccine timeline. Scroll up to see it above or visit this link.
  • On Dec. 15, Elliot Hospital ICU nurse Heidi Kukla became the first person in New Hampshire to be vaccinated for COVID-19.
  • As of Dec. 5, the state has temporarily paused daily monitoring of people who might have been exposed to COVID-19.
  • On Dec. 3, the state updated its guidance for quarantining, reducing the 14-day window for most situations down to 10 days.
  • On Nov. 19, the governor announced a statewide mask mandate, effective Nov. 20. Anyone over the age of 5 in a public space, indoors or outdoors, who cannot physically distance from others not in their own household shall wear a mask, according to the emergency order. This remains in effect.
  • On Nov. 12, state officials said they will no longer conduct contact tracing for every single case and instead will focus on certain populations.
  • On Oct. 2, health officials began adding antigen test results to the totals because the rapid tests are becoming more widely used. That resulted in an apparent spike in positive cases, as positive antigen tests from over the course of the pandemic were added. One additional death and five hospitalizations related to positive antigen tests were also added.
  • On Sept. 24, the governor said that all restaurants can move tables closer than 6 feet as long as there is a protective divider in place between tables.
  • On Aug. 21, the governor announced that all New Hampshire restaurants could open at 100% capacity, though they still must following distancing and mask guidelines.
  • On Aug. 11, the governor announced that masks would be required at all scheduled gatherings of 100 or more people.
  • On June 15, the stay-at-home order expired and was replaced by a safer-at-home advisory. A key difference is that the requirement that all groups must be fewer than 10 people is no longer in place.
  • On May 22, officials announced the state’s first case of MIS-C in a child.
  • On March 23, officials announced the state’s first death of a patient diagnosed with COVID-19.
  • On March 13, the governor first declared a state of emergency, which ensures resources will be ready so the state is able to respond quickly, as needed. This has been renewed and is currently active. It needs to be renewed every three weeks while an emergency is ongoing in order to remain active.

SYMPTOMS

Officials said symptoms might appear in as few as two days or as long as 14 days after exposure.

Symptoms include:

  • Fever or chills
  • Cough
  • Difficulty breathing or shortness of breath
  • Fatigue
  • Muscle or body aches
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

According to the CDC, emergency warning signs for COVID-19 include:

  • Difficulty breathing or shortness of breath
  • Persistent pain or pressure in the chest
  • New confusion
  • Inability to wake or stay awake
  • Bluish lips or face

PROTECTING YOURSELF

The best way to prevent illness is to avoid being exposed to the virus, officials said.

Here are ways to protect yourself:

  • Frequent hand-washing with soap and water (or hand sanitizer with at least 60% alcohol) for at least 20 seconds
  • Avoid close contact through social distancing (at least 6 feet from someone)
  • Stay home

PREVENTING THE SPREAD

There are steps that can be taken to avoid spreading coronavirus:

  • Covering coughs and sneezes and then washing hands afterward
  • Stay home and avoid public places when sick
  • Avoid being within 6 feet of a person when sick
  • Disinfect frequently touched surfaces
  • Avoid sharing drinks, smoking/vaping devices or other utensils or objects that may transmit saliva
  • Wear cloth face coverings when near other people in public

CURRENT TESTING PROCEDURES

Testing is available for active COVID-19 infections and for antibodies, which would indicate evidence of past infection.

All Granite Staters, whether they have symptoms or not, are allowed to get tested and are being encouraged to do so. Visit this link to learn more.

The FDA has also approved a test for COVID-19 that you can take at home. The test kits are available for purchase on Amazon with a turnaround time for results of 24 to 72 hours after the sample is shipped and received.

QUARANTINING

The quarantine period is now 10 days, down from 14 days as was previously required by the CDC and state health officials. Here’s what you need to know about each situation that might lead to being asked to quarantine or isolate.
