911: Lone Star actor Tyler Sanders died from an accidental overdose of fentanyl

911 Lone Star actor Tyler Sanders died from the effects of fentanyl, his autopsy has revealed.

The Fear The Walking Dead star was found dead in his LA home in June at the age of 18, with the Los Angeles County Medical Examiner noting that his death was accidental.

Tyler, who had been nominated for a Daytime Emmy for his role in the Prime Video series Just Add Magic: Mystery, landed guest roles on shows including 911: Lone Star, and had been actively posting on social media before his death.

In the report, obtained by TMZ, the Medical Examiner said that Tyler sent a text message to a friend the night before his death, telling them that he was using fentanyl.

It also stated that he did not answer phone calls from his friend after sending the message about his drug use.

The coroner also stated that Tyler had no known medical problems, but did have a history of drug use, including heroin, cocaine, LSD, mushrooms and Xanax. 

Police found a plastic straw and white powder in the room where he died, the report stated.

It comes after Tyler was found dead at home at his home in LA, after police responded to a call concerning a male who was found not breathing.   

Responding officers found Sanders alone at the home and shortly after pronounced him dead.

At the time, the police did not suspect foul play and found no signs of suspicious circumstances. 

On its website, the LA County’s Medical Examiner-Coroner said Sanders’ cause of death was ‘deferred,’ meaning ‘pending additional investigation.’ 

Aside from his role on the YA spinoff of Just Add Magic, the teen actor was best known for guest roles on the small screen in 911: Lone Star, The Rookie and Fear the Walking Dead.

A representative for Sanders, Pedro Tapia, told the site that the cause of death would be investigated. 

‘Tyler was a talented actor with a bright future,’ Pedro Tapia said in a statement. ‘He comes from a wonderful family, and we ask that you respect their privacy at this time.’

Sanders kept a fairly active presence on social media, including a Facebook page where he was often seen at home or on vacation with his large family. 

His last Instagram post was from five days ago where he was seen wearing a blue suit while on a patio in Vail, Colorado. The caption read, ‘Styling.’ 

He had been in Vail with his family as they were seen white river rafting. ‘Here’s a quick before and after of my family white water rafting,’ he wrote in his caption as his family was seen paddling, then in the next photo, hit with water.

Sanders landed an Emmy nomination for role on the Amazon series, Just Add Magic: Mystery City.

The young actor started working professionally at age 10 after working on stage in school projects. His first role was in 2015 on the show JLW Academy as himself.

Next he landed the role of Young Jake Otto on Fear The Walking Dead in the 2017 episode titled TEOTWAWKI which led to the role of Siggy in the TV movie What About Barb?

He played Logan Hawke in the TV series The Rookie in 2018. In 2019 he starred as Eli in the Reliant which won critical acclaim.

Magic role: In 2019 he played Leo in Just Add Magic. He had a bigger role, again as Leo, in the spinoff Just Add Magic: Mystery City in 2020

He started very young: He began acting at 10, including a guest part on Nathan Fillion’s police drama The Rookie

Also in 2019 he played Leo in Just Add Magic and the following year he starred in the TV movie Escaping My Stalker. 

He had a bigger role, again as Leo, in the spinoff Just Add Magic: Mystery City in 2020.

On 911: Lone Star he played Brian in the 2022 episode Impulse Control.

Tyler has the movie The Price We Pay in post production waiting for release. 

In the past he had worked with actress Leah Remini, who followed him on Instagram.

‘Face Off’ Laney Chantal dead at 33 after accidental overdose

Celebrity makeup artist and former reality TV star Laney Chantal died this week of an accidental drug overdose. She was 33.

Chantal, whose real name is Alaina Chantal Parkhurst, died Monday in Milford, Michigan, her family confirmed in a public obituary, writing that she had “struggled with various mental illnesses throughout her adult life.”

A prominent FX makeup artist and mask designer, Chantal came into the spotlight after appearing on the SyFy Network’s “Face Off” competition show in 2013 for its fifth season. She voluntarily withdrew from the reality show, about special-effects makeup artists, earning the fifth-place spot.

Chantal worked with several big-name stars following her appearance on the show, including Bella Thorne and Marilyn Manson. She even created the “Head FX Makeup” for rapper Lil Nas X for his controversial music video “Montero (Call Me By Your Name)” in 2021, which won the Video of the Year award at MTV’s VMAs.

Fellow makeup artist Gabriella Accarino remembered Chantal as her “sister and soulmate.”

“She was a genuine, sensitive, talented, and beautiful soul,” Accarino wrote. “Laney’s art and her spirit are irreplaceable, and she made the world a better place. She was truly one of a kind.”

According to her family, Chantal moved from Michigan to Los Angeles after graduating high school in 2007 to attend the Cinema Makeup School, where she earned a certification to become a special effects makeup artist.

“Alaina put her whole self; body, mind and soul into her jobs, no matter the size,” her obituary said. “Her artistic ability and visions quickly set her apart from other artists in her field and her portfolio blossomed.”

Her family did not provide additional details on her death other than it was due to “an accidental drug overdose.”

Six months before her death, Chantal posted a photo to Instagram of a pink coffin being buried.

The Parkhurst family asked for donations to be made to The Brain & Behavior Research Foundation, noting the struggles its daughter experienced with mental health throughout her life.

Everything New in iPadOS and iOS 16.2 Beta 1: Freeform App, Stage Manager External Display Support, Accidental Emergency SOS Reports and More

Apple today provided the first betas of iOS 16.2 and iPadOS 16.2 to developers, introducing even more new features that have been promised for the iOS 16 update.

We’ve outlined all of the changes found in the iOS 16.2 and iPadOS 16.2 betas so far in this guide.

Freeform App

Apple debuted a new cross-platform Freeform app in iOS 16.2, iPadOS 16.2, and macOS Ventura 13.1. Designed for collaboration, Freeform can be used for jotting down notes, sketching, drawing, saving links, and more.


Multiple people can work on the same Freeform document, with updates synced for all participants in real-time. Apple says that Freeform can be used for sketching projects, designing mood boards, and brainstorming ideas, with the app serving as a creative space.

Stage Manager External Display Support

With iPadOS 16.2, Stage Manager on M1 and M2 iPads can be used with external displays once again. This is functionality that Apple removed in the first version of iPadOS 16 in order to expand ‌Stage Manager‌ to A12X and A12Z iPad Pro models.


With external display support, ‌Stage Manager‌ supports up to eight apps for multitasking purposes, rather than four. This functionality is limited to ‌M1‌ and ‌M2‌ iPads.

New Home App Architecture

Paired with HomePod 16.2 software, iOS 16.2, iPadOS 16.2, and ‌macOS Ventura‌ 13.1 add support for an updated Home app architecture that comes following the addition of the Matter smart home standard.


Apple says that the new Home app architecture will bring faster, more reliable performance in homes with many smart accessories, but it requires every device that accesses the home to be running the iOS 16.2, iPadOS 16.2, ‌macOS Ventura‌ 13.1, tvOS 16.2, and watchOS 9.2 betas, along with the HomePod 16.2 beta software.

On devices that support ProMotion, Apple says that SwiftUI animated layout changes will support a 120Hz refresh rate, functionality that was previously missing.

Unintentional SOS Calls

Apple appears to be collecting data on accidental SOS calls with the update, and one user has reported an Apple inquiry about the call that collects system data.

Software Updates

Apple has made a small tweak to the Software Updates section of the Settings app, which is available under General. The text of the software version that’s installed is now bolder, making it more apparent.

Anne Heche’s death ruled accidental after fiery car crash | Anne Heche

Anne Heche died from inhalation injury and burns after her fiery car crash and the death was ruled an accident, according to coroner’s results released on Wednesday.

Heche, 53, also had a fractured sternum caused by “blunt trauma”, according to information on the website of the Los Angeles county medical examiner.

A full autopsy report was still being completed, the coroner’s office said.

The actor was removed from life support Sunday at a burn center. She was injured when her car jumped a curb and smashed into a west Los Angeles home on 5 August. The car and the home burst into flames. Only Heche was injured.

Heche suffered a “severe anoxic brain injury” caused by a lack of oxygen, according to a statement released last week on behalf of her family and friends.

She was declared brain-dead but was kept on life support until her organs could be donated.

Detectives looking into the crash had said narcotics were found in a blood sample taken from Heche. However, police ended their investigation after she was declared brain-dead.

The coroner’s office listed 11 August as her date of death.

Heche first came to prominence on the NBC soap opera Another World in the late 1980s before becoming one of the hottest stars in Hollywood in the late 1990s. She was a constant on magazine covers and in big-budget films opposite actors including Johnny Depp and Harrison Ford.

Ivana Trump’s death ruled accidental by medical examiner

Trump, a longtime businessperson, died in her home in New York City on Thursday at age 73. She was the mother of Donald Jr., Ivanka and Eric Trump.

The New York Police Department had said Thursday there did not “appear to be any criminality” related to her death. According to a statement, police received a 911 call about an “aided individual” at about 12:40 p.m. ET and found a “73 year-old female unconscious and unresponsive.”

The Fire Department of New York said it responded to a report of an individual suffering cardiac arrest at the residence, with the time and place of that response matching the location the NYPD associated with Trump. The fire department said the victim was dead on arrival.

EMS, police said, pronounced Trump dead at the scene.

Raised in communist Czechoslovakia, Ivana Trump partnered with Donald Trump on some of his most prominent real estate projects. The two divorced in 1992 in the aftermath of his tabloid affair with Marla Maples — who later became Donald Trump’s second wife and mother of their daughter, Tiffany.

After the divorce, Ivana Trump married and divorced twice more while maintaining a jet-setting, globe-trotting lifestyle.

Her children on Thursday paid tribute to her business acumen, humor and passion.

Ivanka Trump tweeted that she was “heartbroken by the passing of (her) mother,” adding: “She lived life to the fullest — never forgoing an opportunity to laugh and dance. I will miss her forever and will keep her memory alive in our hearts always.”

Elden Ring Patch Makes Radahn Boss Harder After Accidental Nerf

One of Elden Ring’s toughest bosses just got harder, again. FromSoftware released a small patch overnight to buff Starscourge Radahn after the game’s massive 1.03 patch accidentally weakened some of the demigod’s attacks. Some Elden Ring players are already rejoicing, because if there’s one thing they love it’s getting mercilessly stomped.

The unofficial boss of Redmane Castle, Starscourge Radahn pelts you with arrows, multi-hit combos, and a range of gravity energy attacks to punish greed and sloppiness. He’s already become a legendary meme within the Elden Ring community for being both amazing and utterly bullshit. If you faced him between March 17 and yesterday, however, you weren’t facing him at full strength thanks to a mysterious bug. Patch 1.03.3, which went live April 4 on console and PC, corrects that.

“Fixed a bug in the balance adjustment of the boss ‘Starscourge Radahn’ in update 1.03, in which the power of some attacks was unintentionally reduced,” the note reads.

While FromSoftware’s patch notes remain vague, players were able to pick out some changes to Radahn shortly after update 1.03 originally came out. Notably, damage from melee and some other attacks appeared reduced, and his hitboxes for connecting with attacks were also smaller. No longer.

There are a couple of unique aspects to the Radahn fight, chief among them the fact that a bunch of famous warriors within the game’s fictional universe are summoned to fight alongside you. This helps distract Radahn so players sneak in damage without immediately getting clobbered. It also means Radahn is built to take on an entire squad. There are also classic Souls moments like the way he vanishes into the air midway through the fight only to crash back down to earth moments later raining death and destruction.

But because of the accidental nerf, many players recently trounced Radahn without realizing they’d been given an extra helping hand. “I beat him 2nd try on first playthough… i didnt know he was nerfed.. i thought i was lucky,” wrote one player on the Elden Ring subreddit. “Defeated him on my third attempt, I also didn’t know this lol,” wrote another.

Some players considered Radahn much too easy, especially on second playthroughs. “40 years from now: ‘gather, grandchildren, and hear the tale of my epic duel with pre-nerf Radahn, the Scourge of the Stars,’” quipped one shortly after the nerf was discovered. Now whether they wanted to or not, everyone else can bask in the glory of the Radahn beatdown as it was apparently intended.

Accidental injection death of wrong drug: Ex-nurse on trial

NASHVILLE, Tenn (AP) – The attorney for a former Tennessee nurse on trial in the death of a patient accidentally injected with a paralyzing drug told jurors Tuesday the woman is being blamed for systemic problems at Vanderbilt University Medical Center.

RaDonda Vaught, 37, is facing a charge of reckless homicide for administering the drug vecuronium to 75-year-old Charlene Murphey instead of the sedative Versed on Dec. 26, 2017.

Murphey had been admitted to the hospital two days earlier after developing a headache and losing vision in one eye. On the 26th, doctors ordered a PET scan to check for cancer, but Murphey was claustrophobic and asked for medication to control her anxiety, according to testimony.

Vaught could not find Versed in an automatic drug dispensing cabinet because it was listed under the generic name midazolam. Instead, she used an override mechanism to type in “VE” then grabbed vecuronium, according to court records. Vaught left the imaging area after injecting the drug, but minutes later another employee noticed Murphey was unresponsive.

The family removed the woman from life support in the early hours of Dec. 27.

Nashville Assistant District Attorney General Debbie Housel said during opening statements Tuesday that the nurse ignored warning labels on the medication and didn’t notice the medication she chose was very different from the one needed.

“RaDonda Vaught recklessly ignored everything she learned in school” when she administered vecuronium, a drug sometimes used in executing prisoners, the prosecutor said.

“Vecuronium is only given when someone is intubated because it paralyzes the body,” Housel said. “A person can’t breathe, can’t move, can’t shout for help.”

In addition to the override, Vaught also failed to scan the medication against the patient’s medical identification bracelet, Housel said. She added that the drug she chose was a powder that had to be reconstituted, instead of a liquid.

Defense attorney Peter Strianse told the jury the hospital was at least partially to blame for Murphey’s death.

He said that in 2017, problems with a new electronic records system led to delays in communication between the pharmacy and the hospital’s automatic drug dispensing cabinets. This often forced nurses to override the system, he said. There also was no scanner for the medication in the imaging area of the hospital where the accident happened.

Vaught admitted her error as soon as she realized it, and the state medical board initially took no action against her. The finger-pointing only began after the Centers for Medicare and Medicaid Services learned of the error and made a surprise inspection at Vanderbilt, according to Strianse.

“This was a high-stakes game of musical chairs and blame seeking,” Strianse said. “When the music stopped, there was no chair for RaDonda Vaught.”

Testifying on Tuesday was Murphey’s daughter-in-law Chandra Murphey, who cried as she recalled Charlene Murphy’s last days.

The two had been preparing Christmas dinner on Dec. 24, 2017, when the older woman started complaining about her vision. Chandra Murphey convinced her to go to the emergency room, where they found a brain bleed. Murphey was transferred to Vanderbilt’s intensive care unit but was getting better before the accident, Chandra Murphey testified.

She was at the hospital with her mother-in-law when she was taken downstairs for the PET scan and saw her again when she was brought back upstairs, surrounded by the doctors who were trying to save her life.

“How in the world do you take someone down for a PET scan and bring her back like this?” she asked. “They basically took her down fine and brought her back dead.”
